All Risks Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Take photographs of the damage for inspection by EQI's appointed representative. Do not dispose of damaged item(s), unless agreed by us.
  3. Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.
  4. Submit all relevant documents like repairer's quotation, original receipts and invoices, within 30 days to us.
  5. Do not admit liability or make any settlement with any third party without our prior consent.

Burglary / Cash in Transit Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Notify the police immediately and submit a copy of the police report to us.
  3. Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.
  4. Provide us full details of the loss, including the full value of each article lost supported by documentary proof.

Contractors' / Erection All Risks Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Notify the police immediately and submit a copy of the police report to us.
  3. Provide us full details of the loss supported by documentary proof.
  4. Assist and co-operate with EQI's appointed representative.
  5. If any writ of summons, letter of claim or other legal processes have been issued or commenced against the policyholder/ insured, submit these documents to EQI immediately.
  6. Assist EQI's appointed representative or adjuster to:
    a) settle any claim;
    b) resist any claim; and
    c) institute proceedings.

Fire &  Extraneous Perils Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Submit all relevant documents like repairer's quotation, original receipts and invoices, within 30 days to us.
  3. Take photographs of the damage for inspection by EQI's appointed representative. Do not dispose of damaged item(s), unless agreed by us.

Home Insurance

Marine Cargo Insurance

Please adhere to the following procedures to expedite claims, when loss / damage to cargo is discovered at the Sea Port/ Airport:

  1. Do not authorise delivery of cargo to the final warehouse. Notify EQI and forwarders/ carriers immediately for a joint survey immediately.
  2. Submit a written notice of claims immediately to the carriers, forwarders, Port Authorities, bailees and other parties concerned if there is any missing or apparently damaged cargo.
  3. Before delivery from the Sea Port/ Airport, ensure that all container seals for containerised cargo are intact.
  4. If goods are in doubtful and apparently damaged condition, do not give clean receipts, except under protest. Take steps to minimise the loss or damage.
  5. EQI’s appointed representative or adjuster will contact you and assist you with the claim settlement. Do supply them with the necessary information and documents.

Motor Insurance

In case of accident:

  1. Do not panic. Stay calm and exchange particulars, such as vehicle registration number, full name as in NRIC, address and contact details.
  2. If there is any injury, call the police and do not remove the vehicle.
  3. If possible, take photographs at the scene of the accident showing the position of the vehicles and the damages. Forward the photographs to EQI as soon as possible.
  4. Do not admit liability or make any settlement with regards to the accident.
  5. Ask for the name, address and contact numbers of the independent witnesses, if any.
  6. Proceed to our Reporting Centres or any of our authorised workshops to report the accident within 24 hours or the next working day of the accident regardless of whether you are claiming under your own policy.
  7. For windscreen repair due to an accident, you have to file an Own Damage claim and Excess will be applied. Please proceed to our authorised workshops listed in your policy.
  8. You are required to lodge a police report if the accident involves:
    1. injury to any parties
    2. a government vehicle or government property
    3. a foreign vehicle
    4. pedestrian or cyclist
    5. "hit-and-run" case
  9. If you are making a claim under your own policy, request the workshop to submit a quotation for repairs to EQI.

Personal Accident / Medical Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. For Personal Accident, download this claim form and submit the completed form to us within a month.
  3. For Medical Insurance, download this claim form and submit the completed form to us within a month.
  4. Submit to EQI all relevant documents like original medical bills, receipt and invoices.
  5. A medical report may be required.

Public Liability Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Download the claim form and submit the completed form to us within 7 days.
  3. Notify EQI immediately and submit any writ of summons, letter of claim or other legal processes have been issued or commenced against you.
  4. Do not admit liability or make any settlement with any third party without our prior consent.

Work Injury Compensation Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.
  3. Notify the Ministry of Manpower (MOM) of the occurrence using iReport at www.mom.gov.sg in accordance with the Work Injury Compensation Act and the time limits set out by Legislation.
  4. Submit to EQI a copy of the iReport within 10 days.
  5. Submit all relevant documents like original medical bills, receipts, invoices and the police report.
  6. Notify EQI immediately and submit any writ of summons, letter of claim or other legal processes have been issued or commenced against you.

Foreign Worker Medical Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.
  3. Submit all relevant documents like original medical bills, receipts, invoices and thepolice report.
  4. Notify EQI immediately and submit any writ of summons, letter of claim or other legal processes that have been issued or commenced against you.

Travel Insurance

  1. Notify your agent, broker or EQI about the incident immediately.
  2. Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.
  3. Submit the photographs of the damaged item(s) together with the original invoice(s) and completed claim form.
  4.  For luggage delay/ damage claims, submit to EQI the Property Irregularity Report/ Property Damage and Luggage Tags together with completed claim form.
  5. Submit the original medical receipt(s) together with completed claim form.
  6. For travel delay claims, submit to EQI the relevant Airline’s confirmation letter stating the reason and duration of delay together with completed claim form.
  7. Submit to EQI a copy of the police report if the loss occurred while not in the custody of the airlines and Transport Authorities together with completed claim form.
  8. Assist and cooperate with EQI’s appointed representative or adjuster.
